Dosage Form Packaged in a cardboard box containing 2 blisters of 15 capsules each along with a patient information leaflet.

Composition

Each capsule contains:

• Vitex agnus-castus (Chaste Tree Fruit) Dry Extract: 100 mg
• Fenugreek Seed Dry Extract: 50 mg
• Ginger Rhizome Dry Extract: 50 mg
• Iron (Ferrous Sulfate): 7.5 mg
• Ascorbic Acid (Vitamin C): 100 mg
• Standardized to contain 1.8% diosgenin.

Indications

Simognus Capsules are used to help alleviate premenstrual syndrome (PMS) symptoms, including:

• Irritability
• Anxiety
• Depression
• Fluid retention
• Food cravings
• Breast tenderness
• Headache
• Bloating

Dosage and Administration

Take two capsules daily starting 14 days after the onset of menstruation until the first day of the next menstrual cycle.

Pharmacological Effects and Mechanism of Action

Vitex agnus-castus: The therapeutic effects of Vitex agnus-castus are attributed to its indirect impact on neurotransmitters and hormones. Its active components inhibit prolactin secretion, which can prolong the luteal phase of the menstrual cycle. Hyperprolactinemia and luteal phase defects are significant contributors to PMS and cyclical breast pain. Clinical studies demonstrate that the fruit extract alleviates PMS symptoms, particularly breast pain and inflammation, and can reduce hot flashes, fluid retention, depression, and other menopausal symptoms.

Fenugreek and Ginger Extracts: These components help reduce menstrual bleeding. Ginger inhibits prostaglandin synthesis, providing positive effects during menstruation.

Ascorbic Acid (Vitamin C): Enhances the absorption of iron in the body, compensating for iron loss during menstruation.

Precautions

• Gallstones: Avoid use in patients with gallstones.
• Surgical Procedures: Discontinue use two weeks before any surgical procedure due to the potential risk of bleeding.

Adverse Effects

• No adverse effects have been reported with recommended doses.
• Rarely, Vitex agnus-castus may cause:
  • Pruritus
  • Headache
  • Gastrointestinal disturbances (e.g., reflux, diarrhea, nausea, vomiting)
  • Insomnia
  • Weight gain
• Discontinue use if any of these symptoms occur.

Drug Interactions

Avoid simultaneous use with:

• Dopamine agonists (e.g., amantadine)
• Dopamine D2 antagonists (e.g., metoclopramide)
• Antipsychotics
• Contraceptives
• Estrogen therapies
• Anticoagulants and antiplatelet agents

Use During Pregnancy and Lactation

• Contraindicated during pregnancy and breastfeeding.
